The plot has four separate storylines. One focuses on married Sun and Jin Kwon on the island, as they debate on whether to defect to the rival faction of survivors led by John Locke. On the Kahana, Sayid Jarrah and Desmond Hume learn more about the intentions of the freighter's crew. In two separate off-island storylines, both Jin and Sun are caught up in pregnancy-related troubles.
Before the premiere of the fourth season, the principal cast of Lost called "Ji Yeon" the most shocking of the first seven episodes.[1] The episode was filmed in late October and early November of 2007.[2]

The episode presents four storylines, three of which follow Jin (Daniel Dae Kim) and Sun (Yunjin Kim). The other storyline focuses upon Sayid Jarrah (Naveen Andrews) and Desmond Hume (Henry Ian Cusick) on the freighter Kahana.
On the island, Sun becomes increasingly worried about her pregnancy, and Juliet Burke's (Elizabeth Mitchell) admission to her that women who conceive on the island die during their second trimester. Because she fears that rescue will not come quickly enough, she arranges to defect with Jin to the rival faction led by John Locke (Terry O'Quinn). Juliet tries to stop Sun from leaving by telling Jin that Sun had an affair. Their dispute is settled when Sun realises that waiting for rescue will be more advantageous.
On the Kahana, Desmond and Sayid are still waiting to speak to the captain of the freighter, Gault (Grant Bowler). When they are taken to see him, Regina (Zoƫ Bell) jumps off the side of the ship, but Gault dismisses it as "a heightened case of cabin fever", and losing her is better than losing several more in saving her. He takes them into his cabin, and explains the reason that Charles Widmore (Alan Dale) is so intent on finding Ben Linus (Michael Emerson) is because faking a plane crash would require tremendous resources. After their meeting, Desmond and Sayid are relocated to a different part of the ship, where they meet Michael Dawson (Harold Perrineau, Jr.) working as a janitor under the alias "Kevin Johnson".
The episode presents two separate off-island storylines, both involving pregnancy. In flashbacks, Jin's journey to a hospital is marred by several unfortunate incidents, including his cell phone being broken. When he arrives at the hospital, he sends his employer's wishes to a Chinese ambassador. In flashforwards, Sun gives birth to a girl, named Ji Yeon. She is later visited by Hugo "Hurley" Reyes (Jorge Garcia), who takes her to Jin's tombstone to pay their respects
